Bruises commonly occur in children and are most often the result of a minor accidental injury. However, bruises can also signal an underlying medical illness or an inflicted injury maltreatment. Although bruising is the most common manifestation of child physical maltreatment, knowing when to be concerned about maltreatment and how to assess bruises in this context can be challenging for clinicians.

Based on current literature and published recommendations, this practice point will help clinicians to distinguish between accidental and inflicted bruises, to evaluate and manage bruising in the context of suspected child maltreatment, and to evaluate for an underlying medical predisposition to bruising.

This digital download provides a fuller examination of the research related to the clinical assessment of bruises—the 1st portion of this clinical guide. I receive a lot of questions about determining the age of bruises. Although the research has shown that determining the age of bruising by clinicians based on color provides consistently inaccurate results, with poor interrater reliability , I still find that some are loathe to turn their backs on this highly unreliable assessment technique.

So I have provided an overview of the literature below, with articles split into 2 categories: those that address the attempt to age bruises based on color in a routine clinical environment Clinical Assessment of Bruises and those that have a much more high-tech approach Laboratory Assessment of Bruises. These articles address aging bruises using equipment and mathematical models not typically seen in our routine clinical practices.

The articles below address bruising in a wide range of populations.

Each patient’s age was calculated by subtracting the date of birth from the date of the visit. To exclude data that may have been inaccurately recorded, as well as.

Once the bruise is formed, the body reacts by activating the inflammatory response. There are 2 types of inflammatory cells that migrate to the site of the bruise. Neutrophils are a type of white blood cell that arrive first. Then the macrophage cells, which are larger white blood cells, arrive later. Their arrivals to the injury occur at certain time intervals. Therefore, when the inflammatory response is activated, injured tissue can be analyzed for the presence of these cells to interpret the age of the bruise.

In one study , tissue analysis compared 1-day old bruises to 5-day old bruises. Neutrophils are not present in normal skin. Findings of the study demonstrated that when an injury occurred and the inflammatory response was activated, neutrophils migrated to the injured tissue at approximately 4 hours after the initial bleeding: This indicated fresh bleeding. Macrophage cells began to migrate as early as 3 hours and up to 9 hours after initiation of a bruise with continued presence and migration at days.

With bruises aged days old, the macrophage cells had infiltrated the injured tissue with the most intense infiltration occurring in the 5-day old bruises. Therefore, according to the study, by analyzing in detail, the tissue changes in bruises every day from appearance until their healing and in conjunction with tissue analysis will support the precise determination of the age of the injuries themselves. Attorneys who are litigating homicide cases can utilize this information to assess which of the injuries sustained occurred just prior to death or several days prior.

While this is still not common practice during autopsies, it is likely that we will see more and more histological samples of bruised areas included in the reports.

Bruises: Is it a case of “the more we know, the less we understand?”

The present invention relates to determining the age of skin bruises based upon their reflectance spectra. Determining the age of injuries on a victim’s body is an important aspect of forensic medicine, for example in cases of abuse. Bruises may be observed directly or photographs may be inspected. However, interpretation of the finding is purely empirical and relies strongly of the skills and experience of the observer, and might be disturbed by factors like e.

Forensic nursing experts and other health professionals often testify regarding the injuries women and men receive from violence. Bruise discoloration can provide an opportunity for health professionals to grossly estimate the age of impact and determine if it is consistent with a given reported history.

However, there is much confusion in the literature and clinical practice as to the accuracy of dating bruises. This article will demonstrate that using the color of the bruise as the primary criteria to determine its age is not the best practice. The limited available research and related literature on the topic are flawed and inconsistent. Finally, recommendations will be made for future research and clinical practice.

The dating of bruises can be of crucial interest in clinical forensic medicine, but the macroscopic or photographic evaluation of a bruise has not been regarded as reliable. Several methods have thus been regularly evaluated for this purpose, with reflectance spectrometry yielding promising results. The objective of this study is to evaluate the value of a bilirubinometer as a tool for dating bruises on living victims. A clinical follow-up was carried out on patients presenting a bruise with a known onset.

Using a bilirubinometer, we obtained daily measurements of the bruise bili-bruise and of healthy skin bili-skin.

Included studies assessed the age of bruises in live children less than 18 years old. Excluded: significantly correlated with dating accuracy.

In this issue of the journal the possibility that heme oxygenase activity is responsible for the delay in the appearance of the yellow color in bruises has been investigated [ 1 ]. Bruises in the skin and subcutaneous tissues occur when there has been extravasation of blood following the application of blunt force. Generally the requirements for their formation are injury, compromise of the integrity of vessel walls, and blood pressure [ 3 ].

The most useful bruises in a forensic context are those that have a patterned appearance reflecting the nature of the impacting object. Questions that are commonly asked in court regarding bruises involve their age, the amount of force required for their production, and whether they occurred before or after death.

Forensic scientists have to rely on their own subjective experience when asked to ascertain the age of contusions. Now, however, researchers in Norway have found a far more objective and precise method. Previous research has shown that forensic scientists are wrong roughly half the time when attempting to determine the age of bruises based on their visible colours.

Such a high margin of error is unacceptable, so Norwegian researchers began searching for more reliable methods for dating bruises. In a project that receives funding under the Research Council’s open competitive arena for independent, researcher-initiated basic research projects, the NTNU researchers first studied various bruises using light, then they formulated algorithms for how a bruise progresses over time.

The technology they applied is simple and common: a lamp illuminates the bruise, and a spectrometer measures the reflected light. The true breakthrough, however, came when the researchers worked out a model that predicts how a bruise will develop over time. They were able to conduct controlled studies of bruise infliction — thanks to willing volunteers from among NTNU’s martial arts enthusiasts.

A study was also carried out on older bypass operation patients taking blood-thinning medication.
